Appointed May 29, 1915; report dated July 27, 1915. Commissioner: Mr. Justice Denis Murphy, of the Supreme Court. The commission was appointed to inquire into the cause of and responsibility for the accident of February 9, 1915, in the No. 1 slope of the South Wellington Coal Mines, Vancouver Island, which caused the death of 19 miners. The inquiry and report included an examination of the plans, operation and history of the South Wellington mine.
